VALID HEALTH CARDS ARE REQUIRED FOR ALL APPOINTMENTS
Please be advised that, effective September 1, 2023, we will no longer accept expired or invalid health cards.
If you arrive for your appointment without a valid health card, or with an expired one, your appointment will be canceled and rescheduled.
For all in-person appointments at GFHT and the Geraldton Medical Clinic, patients are required to use their health card to sign in on the tablets. Failure to do so will result in your appointment being rescheduled, as your provider will not be aware of your arrival.
