About Us

Family Health Teams allow Primary Care Providers and other allied health providers to work as a team to focus on keeping our patients and communities healthy. The Greenstone Family Health Team offers comprehensive, multidisciplinary care to members and visitors of the Municipality of Greenstone. Our team consists of Nurse Practitioners, a Registered Nurse, Registered Social Workers, a Health Educator, and Administrative Staff who work in collaboration with the Physicians at the Geraldton Medical Group and Geraldton District Hospital.
