In person and phone appointments are available with all GFHT Nursing Staff to be booked via online booking. (RN, RPN, and NPs)

CHANGE IN RECEPTION HOURS

PLEASE NOTE GFHT AND GERALDTON MEDICAL CLINIC RECEPTION WILL ONLY BE AVAILABLE FROM 9:00 AM TO 12:00 PM, AND FROM 1:00 PM TO 3:00 PM.

YOU MAY HAVE AN APPOINTMENT WITH YOUR PROVIDER OUTSIDE OF THESE HOURS, PLEASE SIGN IN ON THE SELF CHECK IN TABLETS FOR ALL APPOINTMENTS TO ENSURE YOUR PROVIDER KNOWS YOU’VE MADE IT TO YOUR APPOINTMENT.

ANYONE WHO DOES NOT CHECK IN RISKS BEING MARKED AS A “NO-SHOW” BY THEIR PROVIDER. THIS MEANS YOU WILL NEED TO BE RESCHEDULED.

PRESCRIPTION RENEWAL POLICY

Patients requiring renewals of their prescription medication must call their pharmacy and have them send a Prescription Renewal Request to the Medical Clinic or Greenstone Family Health Team on behalf of their Physician or Nurse Practitioner.

Providers have up to 7 days to complete a prescription renewal from the date it is received from the pharmacy.

ALL prescription renewals need to be requested by the patient to their pharmacy AT LEAST 7 DAYS before the prescription runs out. Prescription renewals requests received with less than 7 days may not be refilled in times

Please do not call the clinic to request a prescription renewal, this needs to be done through your Pharmacy. Submitting multiple requests will not decrease the wait time of your prescription

Prescription medications need monitoring and your Provider may ask you to book an appointment to have your medications renewed once the request has been submitted by the pharmacy.

COVID 19 NOTICE

Please be advised The Greenstone Family Health Team will NOT be providing any Home COVID 19 Test Kits and will NOT be offering any COVID 19 Vaccination Clinics.

If you have an appointment are are experiencing any Covid Symptoms, a mask is required. Masks are available in the main entrance, as well as next to the Self Check in Tablets.

If you requires a COVID 19 Home Test Kit, please visit your local Pharmacy.

If you require a COVID 19 Vaccination, please contact the local Public Health Branch at 807-854-0545, or visit https://www.tbdhu.com/districtvaccines for more information.

HOW DOES THE WALK IN WORK?

You must present IN PERSON to the Greenstone Family Health Team between the to sign up for the walk-in. You will be given an approximate time to return to see your provider. Appointments will be given on a first come first serve basis. We suggest coming in for 8:00 or 8:30 to secure your walk-in place as appointments are limited and fill up fairly quickly. Be advised, due to time constraints your appointment will be limited to ONE concern. See the below list of concerns that are appropriate for the walk-in clinic. We will book an appointment for you at a later date for any other concerns.

WHAT TYPES OF APPOINTMENTS ARE APPROPRIATE FOR THE WALK-IN?

Urgent Primary Care Concerns that are best assessed outside of the emergency department. Such as:
– Urinary Tract Infection Symptoms
– Ear, Nose and Throat Concerns
– Cold and Flu Symptoms
– Aches and Pain
– Rashes
– Lumps and Bumps
– High/Low Blood Pressure
– Uncontrolled Diabetes/A1c
– Mental Health Concerns

Welcome to the Greenstone Family Health Team, where we offer comprehensive, multidisciplinary care to members and visitors of the Municipality of Greenstone.  Our team consists of three Nurse Practitioners, a Registered Nurse, a Registered Social Worker, a Health Educator, and Administrative Staff who work in collaboration with the Physicians at the Geraldton Medical Group and Geraldton District Hospital.
